SOCIAL WORKER – MOBILE THERAPY ONLY
Job Overview
We are a multidisciplinary private practice based in Melbourne, offering a range of services to pediatric and adult individuals across Australia. Our offerings include speech pathology, occupational therapy, art therapy, music therapy, physiotherapy, dietetics, and psychology. Our team consists of experienced independent practitioners dedicated to the well-being of families and individuals, working collaboratively to identify concerns and provide effective interventions for each client.
We are actively on the lookout for social workers for our team who will play a crucial role in supporting clients through personalized interventions and addressing their unique concerns.
Tasks & Responsibilities
· Conduct assessments to understand participants’ needs, strengths, and aspirations.
· Develop and implement individual support plans aligned with participant goals.
· Advocate for participants’ rights and ensure their voices are heard.
· Provide counseling and support to address participants’ emotional and social well-being.
· Build strong relationships with participants, families, carers, and other stakeholders.
· Maintain accurate and up-to-date case management documentation.
· Maintain accurate and up-to-date case management documentation, ensuring compliance with reporting requirements (if applicable).
Qualifications & Experience
· Qualification as a social worker with registration with the Australian Association of Social Workers (AASW).
· Bachelor’s degree in social work or any other related field, or Master of Social Work (preferred).
· At least 5 to 6 years of experience working with people with disabilities in a relevant setting.
· Proven experience working with people with disabilities.
· Excellent communication and interpersonal skills.
· Ability to build rapport and trust with participants from diverse backgrounds.
· Demonstrated advocacy skills.
· Well-developed written and verbal communication skills.
· Ability to work independently and as part of a team.
· Commitment to providing high-quality, client-centered care.
· Advanced experience in assessment, crisis-intervention, treatment and case management of children, adolescents and adults with mental health problems.
· Demonstrated experience in assessment, individual counselling and group facilitation.
· Demonstrated effective time management skills and ability to priorities competing workloads within a complex environment

Job Type: Subcontract
Pay: $90.00 – $109.00 per hour
