Submit a Referral
Fill out our secure online referral form or download the PDF version to email it to us. Provide as much detail as possible to help us understand the participant's needs.
Initial Assessment & Intake
Our Intake Team will review the referral within 24-48 business hours. We will contact the participant or referrer to discuss suitability, goals, and funding.
Service Agreement & Commencement
Once everything is aligned, we will draft a Service Agreement. Upon signing, we will match the participant with the right support worker and commence services.
Who Can Refer?
We welcome referrals from a wide network of professionals and individuals, including:
Participants & Families: Self-referrals or referrals made by family members and carers.
Support Coordinators & LACs: Local Area Coordinators and NDIS Support Coordinators.
Medical Professionals: General Practitioners (GPs), Specialists, and Hospital Discharge Planners.
Allied Health Professionals: Occupational Therapists, Physiotherapists, Psychologists, etc.
Community Organizations: Social workers and community health services.

Eligibility & Funding
To access our services, participants must have an active NDIS plan. We currently accept participants whose NDIS funds are:
- ✔️ Plan-Managed: We invoice your chosen Plan Manager directly.
- ✔️ Self-Managed: We invoice you directly, and you claim it from the portal.
- ❌ Please note: At this stage, we are unable to accept NDIA-Managed (Agency-Managed) participants.
